I think getting 'then' confused with 'than' is also a really annoying one. "It was better then that" - then that what?

And using "of" instead of "have" when you're saying "I could/would/should have...". I know it sounds like "of" when you mash the two together (could/would/should've) but it's not.

I don't mind spoken pronunciation and regional dialect. The difference adds richness to life.

However - the only time I really like woefully incorrect grammar in written form is when it's deliberate. For humour, for example, or used by a novelist for authenicity.
