The best one I got in trouble for was testing something a number of years ago and the developer had decided it would be appropriate to enlarge a 2x2 pixel texture to stretch over a big wall.

They cried their cocks off and complained to my bosses because I said in the bug report "this looks poor and ugly." Apparently that was incredibly rude and in no way true.
