err actually most of the morning i have been at work...ive spend about 10 mins on this (i already did the research) and im preping a Roast dinner too so 27 pages pff...try 2 pages :)

Problems associated with Enterobacter sakazakii

1. Classification of E.sakazakii with regards to its pathogenicity

E.sakazakii is a prime example of an opportunistic Enterobacter. Most cases of this bacterium are from premature infants or those with underlying medical conditions, those with autoimmune problems for example.

The main symptoms of its infection caused in Neonates are varied, and can include:

Sepsis: The spectrum of clinical conditions caused by the immune response to a host infection, characterized by systemic inflammation and coagulation in its mild form.
Major Sepsis includes Organ dysfunction, multiple organ failure and death.

Meningitis: An infection of the meningococcal fluid surrounding the brain and spinal cord. Specifically a Bacterial infection is quite severe and may result in brain damage, hearing loss and learning difficulties assuming the patient survives.

Necrotizing enterocolitis: is when the premature intestine reacts abnormally and develops an acute inflammatory response to initial feedings, causing intestinal necrosis. One theory on how NEC can develop connects bowel mucosa damage to bacterial overgrowth when given a carbohydrate source.
The incomplete digestion of Lactose in a premature neonate allows the bacteria to ferment it and overgrow, causing this mucosal damage and inevitably, self digestion.

2. Mode of transmission.

The US Food and Drug Administration found out about a growing body of infections of E.sakazakii in neonates who were fed milk-based powdered infant formulas.
These formulae were produced from a variety of manufacturers in numerous countries including Denmark, the US, Israel, Germany and the UK.

The bacteria are well suited to surviving in this environment until its opportunity to infect arises. It has been found to be heat resistant, so much so that many health professionals have eliminated the use of boiling water to reconstitute powdered milk formulae due to its inability to ensure total destruction of all E.sakazakii.
Also low levels of bacteria in the initial infection are all that is needed to cause all of the diseases listed above.

Infection sources implemented have included Incubators, the birth canal itself and blenders used to reconstitute the powdered milk. With mortality rates as high as 80% this is a serious concern.

3. Natural reservoir.

Until recently the reservoir for this organism has remained elusive, with a study in 1990 failing to identify it from a whole range of sources, including surface water, mud, and soil. Rotting wood, dung, grain, rodents, domestic animals and cattle. Or more importantly raw cows milk.
However, it has been isolated from the gut of the blood-sucking insect Stomoxys calcitrans, taken from farms in North Wales and reared in laboratory culture.
Given the stable fly feeds primarily on Cattle, it seems pretty reasonable that it is possibly this is how it can come into contact with the equipment used to produce milk based powdered formulae and the milk used itself.

4. Methods of Prevention.

The primary method of prevention is a revision in the way we feed newborns in the SCBU (Special Care baby unit) and the Delivery Suite. Since these are heat resistant bacteria simple washing with hot water will not suffice. Most likely a potent disinfectant should be used, or alternatively all equipment should be sterilized after use to prevent any bacteria being able to gain entry and multiply, ready to infect the next baby fed the reconstituted milk.
Production of milk-based powdered infant formulae for medical purposes should be made in a sterile environment, if we put as much care into the preparation of what our newborns feed on in the first few days of their lives, as we did the preparation of microchips and other computer based hardware, a vast majority of cases would have been prevented from the off.
Preparing only small amounts of the formula for each feeding to reduce the rest time at room temperature will also help reduce the amount of time the bacteria have to multiply, if they have managed to survive in the product still.

5. Antibiotic resistance.

Most cases involve infants, however it has been noted that Adults who have been on immunosuppressive therapy or have immunological problems (AIDS for example) can be infected via a wound. One such wound infection was reported to be resistant to Multiple Antibiotics including Methicyllin, only being treated by a prolonged treatment with a cocktail of broad-spectrum antibiotics.
Should a MR strain gain a foothold in infant care units or other places of close proximity to infants we could be seeing a drastic rise in cases of infection with little to prevent it. It seems less likely looking at its history so far, but anything is possible as many Bacterial strains have shown us time and time again.
