Extra mouse buttons are essential imo.

In a DM game, such as Q3, it means you can put your 2 favourite weapons(rail and rox ) under your thumb, for fast switching.
In a realism/gun type game like CS or q3UT, you can have reloasd under your thumb, as well as another important key, such as bandage or sprint.

And in TFC, or Q3f, you can do your grenades on your mouse.

Well i have a 4 Button + Scroller mouse, i use button one to Attack, the 2nd button is my *USE* key (diffusing and hostages etc) and my one near the Thumb (Mouse 3?) is my Voice Comm, Scroller is for my Weapons, and Button 4 isnt assigned
