Tried it 3 years ago for a laugh and it wasn't the case. Don't know if it's changed eversince.

No. But does about 10 seconds after you are uncovered/come out of stealth.

yes
or, to be more specific, it used to work in, i think, 1.85... tried it on my ranger (stealthed) with a caster during one of those Crim-Crau irvr madness sessions.
