I found I had the same probs as megadave, took 5 goes to win. One of the times I started on Madagascar and thought 'cool, got this in the bag' but no, ended up winning with a high visibility engineered disease in Mexico.
Not sure how proud of that I should be.
As Chronic suggested, get your environmental resistances up and deffo dump the early symptom, but I would advise to stay infecting your main country as long as you can to get the points up, then when it gets discovered in others get your transmissions up (waterbourne, airbourne, insect seem best). And I didn't have to go to tier 4, liver failure worked for me (again, I am not sure I feel proud about that). I used a virus, but each to their own. Good fun I found it.
