Job Application help!

Whilst applying for a job.. - a at home reviewing websites job (So therefore self-employed)
A section of the application sheet stated:

[Contractor's Name]
[Address]
Tel:
Fax:
Attn:
Attn:

As I am not a contractor do I still need to fill this out with my home details and as for Attn, is this a abbreviation for attention, and therefore have to put my name again?

If i remember rightly, they say if it is regarding yourself you should fill in every blank line, even if you have already completed it previously. So if its your name or address etc, you fill it in the same as you did previously, instead of writing "Same as above" or somesuch.

With things which aren't for you, you write out in full things like "Not Applicable" each and everytime with care and attention, rather than scibbling "N/A".

I believe it shows you have an attention to detail, least thats what they say...

Hm if you're job a contractor i'd probably pout that as not applicable, are you sure the contractor bit isn't about somebody else, like a reference or something like that? Probably best to phone the company up anonymously just to get a hint of if it's you they're talking about.

I did email them, and they just replied, put your information into this space, so I guess it is yeah.

How are they paying you? What are you doing about income tax?

If you are a contractor, don't they expect you to sort that out yourself?

and Raven.. - Yeah, they do, it's a Irish firm - EURO11.50 a hour or something, and they help you sort out your finances and such out, apparently.
