Maplin will do 'em if you need it fast, if not then the usual suspect online stores. There used to be a Thermaltake one that was highly rated as being better+quieter than the defaults and just stuck on where the old one used to be.
If you want to be daring then you could try a Zalman heat pipe (though you need to check that your card will fit in it properly! >> See Clown's post).

Most fans will stop when you stick your finger in or on them.
however you can buy replacement coolers from people like zalman or coolermaster, and maplins do stock some.
However what i did to mine was i cleaned it out by unscrewing the top and using an air duster, this restored my fan to full working capacity
