Yes i should ask a diatrition(sp?) but i have lost the number :(

In the morning i usually dont have much time for breakfast, so i drink 200 mililitres of OJ then head out to work. The amount of insulin injected is 15 units. Novorapid fast acting insulin.

From what i've been told, 15 units covers about 50 carbohydrates so i have between 15-20 depending on the size of a meal, normally this amount is fine.

however...

in the morning im having 20 carbohydrates from the OJ, with 15 units of insulin, but my sugar level seems to rise...

I suspect the OJ is the problem, it says per 100ML 11 Carbohydrates, of which sugars 11.

But I cannot be sure if this is artificially added sugar, or natural. I am pretty sure its not artificial cause those drinks are really watery and you can taste the sugar!

1) If it's natural sugar from the fruit, would this cause my blood sugar to rise?
2) Is there a way to tell if the Of Which Sugars 11 is artificial or natural?

I'm asking this question early, I think i will try 1 week without OJ in the morning (maybe toast instead) and see what happens, it could be i need more of the 24hour injection...

Would of thought that anything with sugar in it would make the sugar levels rise.

But the thing is, i cant tell if the OJ is 100% natural :(

Gonna avoid it for a week so i can try and figure out if its the cause.

does the juice say "from concentrate"?
if so, that'll be the problem. it means the juice is squeezed, then frozen to be transported, and then water and sugar added to it when it gets to the country where its packaged. a pressed / "freshly squeezed" juice (tropicana for eg) shouldn't cause the sugar spike.

Well, what type of OJ is it? Certain ones are 100% (Tropicana etc)

check the ingredients. if there is anything else listed except oranges then its not 100%. dont know if youve done that, you never mentioned it :)

also on the back of the bottle it will have the actual name of the product. i.e. you think coca-cola is just coca-cola, but on the back it has its name as carbonated soft drink with sugars and sweeteners or something. so if its not 100% orange, it may say orange flavoured shit with added shit.

By no means an answer but my intepretation of it;

The sugar from the fruit could cause an increase, although the sugar in fruit is fructose; which is a simple sugar, meaning it will be digested/absorbed quicker than complex carbs. So if your blood sugar has spiked earlier than normal then its quite possible its the OJ.

Although if i were you i'd ask your dietician :)

When you say "Quicker" it contradicts my statement, but does the fact you also mention Complex Carbs affect things?

Ah yes, fructose is a slow release simple sugar, my mistake.

I only used complex carbs as a comparison; generally they (i.e oats, pasta) are digested for a slower energy release than say eating a bag of sugar. Nothing to do with your situation really.

Most oj's are a bad idea - it will raise your blood sugar far quicker than your "standard" carbohydrates due to the added sugar, nor will it sustain you till your next intake of food. Eventually the insulin will probably catch you up causing a rebound later in the day or the next day - making your bs drop. Things like oj, which do contain sugar are best taken with or after alternative carbohydrate so that the suger is broken down and thus takes longer to take effect.

As said pure fruit juice is better - blend your own it contains no added sugar content :)

Im diabetic myself and have noticed a massive change in bs rates since i went onto the "modern" insulin (on humalog and lantus now). The old humalin was slower acting and didnt cause as much issue with certain quick acting foods.

But back to your question... with the amount of insulin you take comparitive to the amount you consume your bs should not be rising. - you will probably get a quick immediate rise due to the sugar content, but id expect a couple of hours later your bs would drop ? What are your levels like throughout the day ?

Im on the Basal Bolus (sp?) program so i have 1 slow 24 hour injection (Levemir) and 3 fast acting injections (Novorapid). Not sure how much you know on that program but the idea is you take a Novorapid fast acting injection when you have breakfast, lunch and dinner. The idea is its a small fast acting, short duration amount which works against what you've just eaten.

What you mention about "Eventually the insulin will probably catch you up causing a rebound later in the day or the next day" sounds like an ooccurance you'd expect from an injection which is a mixture of fast and slow?

My levels are ok in the day and the evening, but its the morning i sometimes struggle with. It's possible the slow acting 24 hour injection (i take it in the morning) runs out around 4 am (guess), that would mean anything in my system that would require a little bit of insulin would have none available, then that'd mean by breakfast i could be around 8-10 blood sugar lvl.

atm i'm working on assumptions, will have to investigate this OJ and hopefully work it from there.

You might have lost weight, gained weight, changed lifestyle, etc so it's not the OJ that's the problem, but the insulin itself. You may need a doseup, dosedown, you may have become immune to it(CAN and probably WILL happen).

Especially since your 15(counters 50 right?) isn't fighting of a measily 20, there's a problem. You should be dropping not rising. The type of sugar and food is irrelevant as the carbs stay the same. Once more it's not the f*cking OJ.

Oh and....

If ofcourse you're talking about something like a 0.1 rise, 'cause then i'll just have to slap you and tell you to get some f*cking balls.

If it stayes between 4-7(even 8 after big dinner), there's NOTHING to worry about.

Although i'm fully aware a doctor/dietition probably has the answers, i didn't wanna waste their time if i can fix it myself.

I dont think i'm immune to it (not entirely ruling it out) because i still go low if i have too much insulin.

The 15 countering 50 but being affected by 20...This is where im thinking the 24 hour injection may be causing problems, but i'd need to do some testing in the evening and morning to find out what's going on.

Hehe dont worry i'm not complaining about a 0.1 increase

Thanks for the info Tohtori, I plan to monitor my blood as often as possible for a week to see if im going high before bed, if i am, it could be a mixture of that and the OJ causing the problems.

Yeah most likely it's like this, from past experience:

You take the long lasting insulin(this is a stabiliser, to keep insulin at certain amount).
You take the rapid(fast acting) insulin to counter the foodspikes you're eating.

(this depends on WHEN you take the long lasting, let's assume for argument it's at 6am)

24 hour insulin doesn't mean it effect you 24h, all the time, at the same strength. The weak spots are just around 2 hours before and after shot, before it has time to start acting. So from 4am to 8am, you're in a "weak spot".

So, at the time of "juicing" up on OJ, you're probably in the low "dumps" phase of the long lasting, and that's why the rapid acting isn't taking as much effect to the intake of sugar as there's no steady backup.

During the day this isn't a problem since you got the 24h and rapid working together.

Disclaimer: Next part should be 100% absolutely be discussed with doctors, will not work for all.

Solutions to this are:

(since you don't eat much in the morning anyway), is to push the "breakfast" a couple of hours forwards, one or two, depending on if your sugar drops. Basicly kill the OJ, but it's a bad idea to fix your eating to compensate for insulin.

Change the amount of rapid insulin in the morning. This will probably case a rapid drop too in cae of life changes(weight, stress, etc) Also not a good idea.

Change the time you take your long lasting to counter the high in the morning. Probably your best bet. Move it forwards say, 6 hours or so, try to fit the time into a time where you are most less likely to eat. So say, 3pm? Still has a kick in it for the lunch, and has time to restart for the dinner.

Main thing to remember is that your life doesn't change to fit the deseace, you fit the deseace to your life. So you don't have to change one darn thing while you have this, not even drop candy or anything like it.

I think the slow acting and OJ topics are both things to consider. I wont change everything all at once for obvious reasons, but i'm going to see if i can fix this myself. If not, doctor's appointment :p
