Troll Caves, first level.
enter the chamber of the troll chieftain and fight him and his minions
back out of the chamber, closing the door.
recall to the temple
portal back to the caves
open the door, fight minions but chiefy is gone for good heh. [you need his head too, for a minor quest]

OS is win2k, NwN is patched up to 1.22 iirc

I know its not supposed to happen but one possible explanation is this:

In the toolset when you create an area, rather than place individual monsters you can create an 'encounter'. The difference is the encounter is basically a region you draw out on the map, and whenever a player walks into that area monsters are spawned. You specify the types of monster that an encounter can contain, then, depending upon the level of character (or party level if multiplayer) that walks into the encounter region, various monsters from the encounter list are selected and created (you can also specify that certain monsters are unique and can only appear once - for example if its a group of bandits, you'll only want one bandit leader, so you specify him as unique).

The techniques rather nifty if you wanna create a module playable by a range of character levels.
