Single Player : Item's vanishing?

It's happened to me a couple of times. I'll save my single player game, reload it a few hours later, and then realise that a weapon I was using has vanished.
In this case it was a Bastard Sword +2 which, at level 12, I'm sure is a legal item.

Does anyone know why this happens? And does anyone know the command line (if there is one) to insert one back into my single player game?

I've never had this happen personally, but I'm sure it's possible and not just a quirk with your computer or whatever. I always had weird corruptions with the BG games, so I wouldn't put it past them to carry the tradition over.
Infact once, with a thief character in BG2 (thieves turned into Gods the moment they got that "use all items" thing) I realised that the sword from my left hand had vanished into thin air, and it turned out that, checking through my archive of saved games (saved for just such an occasion) it had been missing (through no fault of my own) for quite a considerable amount of time.

The only bug I know of concerning items (well, weapons) in NWN is the equip/los one; if you equip a new weapon, and the old weapon doesn't fit into your backpack, the weapon will drop to the ground, and you won't be able to pick it up again. That's how I recall it anyway. Personally I havn't lost a single item though.
