How and Were the hell do you get it lol!!!

Helm slot items dont start showing up till mid Tier 2 generaly. Along with Cloaks

ofcourse you can just RvR and get the T2 RvR stuff

the "Mask" is the shoulder armor piece. You get the first one from a quest in the forest camp. Next one I found was from the northern High Elf camp in the Shadowlands (Mathrin's Watch). No helms yet though.
Didn't see a shoulder piece on any Renown Gear Merchant yet.

Doesn't show for me when I equip it, much to my annoyance

Silly question, but check on the helm tab on the character screen, there's an option to turn off the visual appearance on helmets there methinks.
