if the char was deleted then restored, then the name becomes available again. there was a case like it on hib/pryd with 2 luri chanters with the same name.

It happened a few months ago, it was a database corruption with names, I made an alt that had the exact name of an existing level 50, you will need to PM a GM what they will do is ask the new character to change the name.

How Many database errors can GoA have in one week?

first Excalibur ME losing there data, then Prydwen going down, now duplicate names in Excalibur.....

wouldn't surprise me if Avalon was having the same prob...

Working as intended for GoA is not the same as working as intended for the rest of the world.. sadly.

Their 'working as indended' is.. if the servers stay up for a whole month... its a miracle.
