Monthly fuel expense statement sheet

Does anyone have an A4-sized (or thereabouts) template for fuel expenses? Just to keep a record of each day's business fuel use, for claiming back at the end of the month, to be kept in the car. Nothing fancy, I can make my own, but if someone has a nice one, ta very much

Have you tried on Microsoft. I know there are a load of templates for such a thing on the office site.

I use a pocket diary in the glove compartment. Easiest way, I find.
