the alter-ego appears upon exiting Vendo Cave, its clickable, you can stick to it, etc etc. Seems like its a 'saved' image of yourself.

Ps: the image is 100% original, no alterations.

Has happened to me too, was very odd, he had right armor & lastname & guild, but wrong firstname
