An array is just a load of disks all copies of each other, any one could have gone missing, in fact that's probably what happened and it wasn't noticed.

To be honest most arrays will give an email/other warning if the disk fails which is what it would of shown up as when it got taken so not to notice it, esp IBM, is very poor indeed.
